Plan Overview
SecureCare SNP (HMO D-SNP) is a Medicare Advantage Health Maintenance Organization (HMO). It covers all of the health care you need, including preventive care, doctor visits, hospital stays, surgery and (if you choose) prescription drugs. In most cases, members must have access care from a network of providers.
Out-of-network coverage is limited to emergency services, urgently needed services, out-of-area renal dialysis and prior authorizations. Members have access to a broad network of health care providers from over 45 counties in Ohio and West Virginia.

There is a Special Election Period for individuals who have Medicare Part A and Part B and receive any type of assistance from the Title XIX (Medicaid) program and/or who qualify for LIS extra help.
There are limits in how often it can be used. This SEP allows an individual to enroll in, or disenroll from, an MA plan once per calendar quarter during the first nine months of the year. This SEP can be used once during each of the following time periods:
- January – March
- April – June, and
- July – September
The Annual Election Period also allows for one change during the months of October-December. This change is effective on January 1.
