The evidence of coverage is a detailed explanation of the health and/or prescription drug benefits and services available to you. It will also include information on how to use your coverage, details about your copay, and your member rights and responsibilities.
Please note: Information about your grievances, coverage and organization determinations (including exceptions) and appeals processes can be found in the chapter titled, "What To Do If You Have A Problem Or Complaint (Coverage Decisions, Appeals, Complaints)."
