For your convenience, full or custom directories can be easily downloaded and saved. If you need help with that, please call our Customer Service Department.

If you would prefer that a provider directory be mailed to you, you may request one at the website link provided above or by calling us. You can also email your request to:  medicare_directoryrequest@healthplan.org.

Please include the following information:

• Full Name
• Mailing Address
• If you are a current member: The Health Plan ID#
